You must have the following documents completely submitted in order to be placed in the lottery.

Pre School Application
Enrollment Selection Form(Google Doc submitted electronically)
Topsfield Pre School Registration Packet
Birth Certificate
Check for $60 made out to town of Topsfield
Proof of Residency

What is needed for Proof of Residency?

Please provide a copy of your deed (you can visit Salemdeeds.com to obtain a copy) if you are the homeowner, or a notarized Topsfield Residency Affidavit with a copy of your lease agreement if not homeowner.
